Piedmont Clinic: 1250 Grand Avenue, Piedmont, CA 94610

This is our main clinic located at Grand and Fairview, about 1/2 mile east of the Grand Lake Theater. We're across the street from an Ace Hardware store and two doors away from a Shell station. The front of the two-story house has a very small wooden sign ("Ann Martin Center") and big painted street numbers ("1250").

From Hwy 580 (East or West):

• Take the Grand Avenue exit.
• Turn left (toward the hills) onto Grand, passing the Grand Lake Theater on the right.
• Go about 0.5 miles, passing through the business district and then passing through the signal at Wildwood Avenue.
• Look for the Shell gas station on the right. We're two doors past the station just
  as Fairview Ave. splits off Grand. Ace Hardware is directly opposite our building.
  (Note that the street numbers change abruptly as you come up Grand Ave.
  ... so avoid using the numbers as a guide.)

Public transportation:

• From downtown Oakland BART, take AC Transit bus #12; the stop is Fairview and Grand.

Oakland Clinic: 3664 Grand Avenue, Oakland, CA 94610

These offices are located on Grand and Weldon, about 1/3 mile from the Grand Lake Theater. The two-story building has yellow-gold bricks. The entrance is on Grand
with an "Ann Martin Center" sign above.

From Hwy 580 (East or West):

• Take the Grand Avenue exit.
• Turn left (toward the hills) onto Grand, passing the Grand Lake Theater on the right.
• After 0.3 miles (and after passing the signal at Mandana), look for our building on the right, at the corner of Weldon and Grand. Enter through the gate on Grand and go up a flight of steps to the second floor. The waiting room door is about half-way down the hall on the right.

  the stop is Weldon and Grand.
